IMPORTANT APPLICATION INSTRUCTIONS

• Open the documents and save to your computer.
• Fill out the application and save again. Be sure and list the project you are applying for. If you are interested in more than one, list them all.
• Attach the reference form and email to the person from whom you are requesting a recommendation. They will need to follow the same procedure. Have them email the reference to the appropriate address below.

We would prefer that you email your forms, but if you are unable to do so please return by mail to the church with attention to the appropriate person.

If you have participated in one of our missions projects in the last two years we can use your previous references. Please note that when you email your application.
